Seeking answers

So our family has an old friend who’s basically family to us he’s 73 and has been an alcoholic his whole life he’s disabled now and can’t leave his house so he basically just lives out his days sitting in a wheelchair smoking cigars and drinking vodka all day I went over to drop off his cigars and noticed he didn’t look as normal as usual looked hella disheveled hair not combed like usual and noticed his hands looking sorta black/cut up almost like liver scaring type cuts and looked slightly pale/yellow in the face and when he sat down I noticed his breathing was a little heavy not like usual probably from all the cigar smoking I was wondering is this a sign of his liver beginning to fail on him or is he just getting old and is still somehow kicking like this? Just looking for answers cause if I have to be the one to admit him to the hospital I will, also when I went to shake his hand like normal, cause he always thanks me for what we do for him I didn’t want to grip too tight cause I genuinely thought something maybe had been wrong with his hands and he looked at me like I knew something was up

Also I’d like to point out that we’ve been wanting him to be admitted in a nursing home/hospital for a while but legally we have no relation to him so we can’t force him and he won’t go unless he has a fall and the ambulance comes and gets him, cause he doesn’t want to not drink and smoke so we just let him live it out
